How Spravato® Works: A Neurological Reset

Spravato works differently than traditional antidepressant medications, acting on NMDA receptors which receive signals from the neurotransmitter (chemical messenger) glutamate. This is one reason that Spravato® has a unique effect on reducing the negativity of depression and suicidal ideation.

Frequently Asked Questions

Traditional antidepressants (SSRIs/SNRIs) target serotonin and can take weeks to work. Spravato® (esketamine) is a nasal spray that targets the glutamate system and NMDA receptors. Because it works on different neural pathways, many patients who haven’t found relief from oral medications see a significant improvement in their depressive symptoms with Spravato.

Yes, Spravato® is FDA-approved for Treatment-Resistant Depression (TRD) and is covered by most major insurance plans, including Medicare. At You First Neuro, our team handles the entire prior authorization and REMS enrollment process to ensure your treatment is accessible and affordable.

The most common side effects include temporary dissociation (a “floating” sensation), dizziness, nausea, and a mild increase in blood pressure. Because of these effects, the FDA requires patients to be monitored by clinical staff for two hours in-office. These sensations are temporary and usually resolve shortly after the observation period.

Every patient is different, but the standard induction phase typically involves two sessions per week for the first four weeks. Following this, Dr. Frank will evaluate your progress to determine a maintenance schedule, which usually reduces to once a week or every other week.

Yes. In fact, that is exactly who this therapy is designed for. SSRIs focus on serotonin, but Spravato® works on synaptic plasticity, literally helping your brain regrow the neural connections that have been damaged by chronic depression. If your “serotonin” pathway isn’t the issue, targeting the glutamate pathway often provides the breakthrough you’ve been looking for.

Spravato® is the brand name of the specific nasal spray medication, while esketamine is the name of the active pharmaceutical ingredient. They are one and the same. Because it is administered as a nasal spray in a highly regulated, FDA-approved clinical setting, Spravato® is specifically designed and cleared for the treatment of Treatment-Resistant Depression (TRD) and Major Depressive Disorder with suicidal thoughts.
